Public Health Modernization Toolkit: Conclusion and Appendices

Conclusion

Improved coordination between public health and health care system partners is essential to the development of a modernized public health system that is capable of improving health for all communities. In confronting a myriad of challenges through the COVID-19 pandemic and beyond, states have developed unique strategies and processes for bringing public health, health care systems, and community partners together to address their biggest health challenges. As states continue to develop initiatives for addressing preventable, costly, and high-burden health conditions at the intersection of public health and health care systems, and take lessons from the COVID-19 pandemic to build resiliency for further public health crises, the strategies and examples outlined in this toolkit can provide a foundation for future endeavors.
