The 2022 National Strategy to Support Family Caregivers emphasizes supporting family caregivers in making needed connections in and out of the health care and long-term care systems to secure needed services and supports. Navigating care is daunting given the complex array of health, social services, and community support — especially in chronically underserved and rural communities. An emerging, but not fully realized, trend in the aging and chronic disease space is to link patients and their caregivers with community health workers (CHWs) to navigate these complex systems. CHWs are trusted community members with shared lived experience and can serve as a key connector to community and social resources.
