This project is a partnership between NASHP and The Health Reentry Project (HARP).
NASHP, in partnership with The Health and Reentry Project (HARP), is hosting a Learning and Action Network to support state officials from across health and human services, behavioral health, corrections, and public safety to exchange information, best practices, and resources as they advance aligned reentry policies and programs. The Learning and Action Network will focus on policies and strategies to meet the health and social needs of individuals leaving incarceration with a particular focus on how Medicaid, health and behavioral health systems, and the corrections system can coordinate to reach these goals. This network will provide a platform for facilitated peer learning through conversations every other month and communication via a listserv between meetings.

State Reentry Learning and Action Network: Building Community Capacity for Reentry Success
This state-only Learning and Action Network (LAN) session will focus on strengthening community capacity to support successful reentry, with particular emphasis on the connections between correctional systems and community-based health and social service providers. As states continue to expand reentry initiatives, effective partnerships with community providers are essential to ensuring continuity of care, timely service access, and long-term stabilization for people returning from incarceration.
Focused on supporting successful reentry, this session will present strategies for fostering long-term partnerships between corrections systems and community-based health and social service providers. Participants will gain insights on establishing referral pathways, coordinating care transitions, sharing information across systems, and aligning roles to improve service access and continuity of care, all with the goal of promoting long-term stabilization for people returning from incarceration.

State Reentry Learning and Action Network: Medicaid Changes Under OBBBA and Their Impact on Reentry — Policy, Practice, and Planning
This state-only Learning and Action Network (LAN) session will explore the implications of the One Big Beautiful Bill Act (OBBBA), or H.R. 1, on state efforts to use Medicaid to strengthen continuity of care for people who are returning to communities from prison or jail. Specifically, the session will examine H.R. 1’s changes to Medicaid beneficiary eligibility and enrollment, including new “community engagement” requirements.
Designed to support state OBBBA implementation and continued progress advancing continuity of care at reentry, this session will identify key policy and operational considerations of the Medicaid eligibility changes for reentry, drawing from the Health and Reentry Project’s (HARP) recent publication. It will also highlight specific strategies states can deploy to leverage reentry work that states are currently leading to effectively implement H.R. 1’s new requirements.

State Reentry Learning and Action Network: Enhancing Collaboration Between Community Supervision and Health Partners to Support Reentry from Corrections Settings
This state-only Learning and Action Network (LAN) session will discuss the roles community supervision partners play in facilitating access to health care at reentry. Aligning systems and engaging front-line partners is essential as states implement policies to connect people to care as they leave correctional facilities. This can promote continuity of care, reduce recidivism, and improve health and public safety.
This session will highlight successful partnerships between community supervision, probation and parole, and key health partners. It will also feature an opportunity for discussion of how community corrections’ role and activities interact with efforts to expand access to health care, including behavioral health care and social services. Specific strategies that will be discussed include training, coordination and referral strategies.

Participants will engage in knowledge sharing and problem-solving to enhance cross-sector collaboration, key stakeholder engagement and exploration of best practices, and identification of challenges and strategies to support justice-involved youth as they transition back into their communities.

Partnerships across Corrections and Health Sectors to Implement Medicaid Changes
Wednesday, May 15, 3:30–5 p.m. ET
This session will focus on opportunities through Medicaid to support individuals returning from incarceration. This will include new and historic changes to the Medicaid program including the youth provisions of the 2023 Consolidated Appropriations Act (CAA), new requirements and opportunities from the 2024 CAA, and Medicaid section 1115 demonstration waivers, as well as other actions states can take through the Medicaid program. Discussion will highlight partnership opportunities across the corrections and health sectors for planning, policy making, and implementation applicable to all states.
