National Care Coordination Standards for Children and Youth with Special Health Care Needs (CYSHCN)

Care coordination is a core component of federal and state efforts to improve health outcomes, reduce caregiver and patient burden, and decrease health care costs for children and adults with chronic and complex conditions.

The National Care Coordination Standards for Children and Youth with Special Health Care Needs (CYSHCN) are designed to help state health officials to:

  • Address state-level challenges, including identifying the needs of CYSHCN
  • Support and engage families of CYSHCN in care coordination
  • Train the care coordination workforce
  • Improve other aspects of quality care coordination systems

States are using these standards in various ways, including identifying areas for quality measurement, guiding strategic planning, and improving their care coordination systems for CYSHCN.
