Successful Partnerships to Address Social Determinants of Health

Thursday, August 22, 10 am – 11:30 am
State health policymakers are collaborating across agencies and at the local level with providers, health plans, and community-based organizations to address the social determinants of health and meet health-related social needs. This session highlights how states can support productive partnerships through health system transformation, managed care contract language, designated partner funding, and organizational restructuring to improve health. Officials from three states, including Michigan and Rhode Island, discuss their partnerships, share strategies, and identify practical ideas to address the social and economic factors that influence health.
