Care Coordination for Individuals with Complex Conditions

States are pursuing a range of strategies to promote more coordinated care and comprehensive support services for individuals with complex health needs. This session will explore states’ care coordination initiatives through three different lenses—primary care, home and community-based services and supports, and services for children with special health care needs—and highlight similarities and differences in service design and delivery.
